Ohio Base: No Harm Reported In Prescription Errors

DAYTON, Ohio (AP) — Officials say errors in prescription refills dispensed to patients at a southwestern Ohio Air Force base pharmacy posed a very small risk to patients and no patients reportedly took the wrong medications.

A statement Monday from Wright-Patterson Air Force Base near Dayton said investigators found eight instances of Tylenol mixed with a muscle relaxant.

The review results focused on improving procedures for maintaining and interacting with the robot used to automatically dispense refills at the Kittyhawk Pharmacy. The statement says quality checks will be run before re-starting the robot, which was determined to be functioning properly.

The review recommended adding more controls to daily operating procedures due to the pharmacy’s extraordinary workflow.

Calls seeking additional information were not immediately returned.

The medications were refilled between April 23 and April 29.
